Tymly Forms Beta Participation Terms
Last Updated: September 23, 2026
These Beta Participation Terms (“Beta Terms”) govern participation in the Tymly Forms private beta program offered by Tymly LLC (“Tymly,” “we,” “us,” or “our”).
By applying for, accessing, or participating in the Tymly Forms private beta, you agree to these Beta Terms.
1. About the Beta Program
Tymly Forms is a pre-release software platform being developed to assist mental health providers and practices with preparing patient forms using AI-assisted clinical documentation review.
The private beta is intended to help us evaluate and improve the platform before broader commercial release.
Participation in the beta is voluntary and does not guarantee continued access to Tymly Forms or access to the final commercial product.
2. Beta Eligibility and Selection
Submitting a beta application does not guarantee acceptance into the private beta.
Tymly may select participants based on factors including practice type, paperwork volume, form types, workflow needs, and the testing needs of the beta program.
We may limit the number of participants or discontinue accepting new beta participants at any time.
3. Pre-Release Software
You understand that Tymly Forms is currently under development.
Beta features may:
-
contain errors or bugs;
-
operate differently than expected;
-
be modified during the beta;
-
be temporarily unavailable;
-
be added, removed, or redesigned; or
-
differ from features included in the final commercial version.
Beta access is provided for testing and evaluation purposes.
4. Provider Review and Clinical Responsibility
Tymly Forms is intended to assist with document preparation. It does not replace professional judgment.
AI-generated content, suggested answers, extracted information, and completed drafts must be reviewed by an authorized healthcare provider before they are approved, signed, submitted, or relied upon.
The participating provider or practice remains responsible for:
-
determining whether information is clinically accurate;
-
correcting or modifying generated content when necessary;
-
determining whether a form should be completed;
-
making all clinical decisions;
-
reviewing the final document; and
-
approving and signing documentation when appropriate.
Tymly Forms does not independently make diagnoses, treatment decisions, disability determinations, employment decisions, or other professional determinations.
5. Protected Health Information
Do not submit patient information or protected health information through the beta application, marketing website, general contact forms, or email unless Tymly specifically provides an approved method for doing so.
During early testing, Tymly may require participants to use synthetic or properly de-identified records.
If a beta phase permits the use of protected health information (“PHI”), PHI may only be submitted through the designated Tymly Forms platform after all applicable privacy and security requirements are in place.
Where required, a separate Business Associate Agreement (“BAA”) must be executed before Tymly receives, maintains, transmits, or processes PHI on behalf of a participating practice.
Participation in the beta application alone does not create a Business Associate relationship.
6. Participant Responsibilities
By participating in the beta, you agree to:
-
use Tymly Forms only for lawful professional purposes;
-
provide accurate information when creating or using your account;
-
safeguard your login credentials;
-
prevent unauthorized access to your account;
-
review AI-generated content before use;
-
use only documentation that you are legally authorized to access and process;
-
comply with applicable privacy, healthcare, professional, and organizational requirements; and
-
promptly notify Tymly of suspected security issues, significant software errors, or unauthorized access.
You may not use the beta to intentionally introduce malicious code, interfere with the platform, bypass security protections, or attempt unauthorized access to another user's or practice's information.
7. Beta Feedback
An important purpose of the beta program is to gather feedback.
We may ask participants to provide information regarding:
-
ease of use;
-
form preparation accuracy;
-
source traceability;
-
workflow;
-
time savings;
-
bugs or technical issues;
-
desired improvements; and
-
overall experience.
Participation may include short surveys, interviews, or other feedback requests.
Unless otherwise agreed in writing, you will not be compensated for providing feedback.
You grant Tymly permission to use suggestions, ideas, recommendations, and other feedback relating to Tymly Forms for purposes of developing, improving, operating, and commercializing the platform without additional compensation or obligation to you.
This does not give Tymly ownership of your patient records, practice records, trademarks, or other proprietary business information.
8. Confidentiality of Beta Features
Because Tymly Forms is a pre-release product, certain features, designs, workflows, screenshots, functionality, documentation, or other information provided during the private beta may not yet be publicly available.
If Tymly identifies information as confidential or non-public, you agree not to publicly distribute or disclose that information without Tymly's permission.
You may provide private feedback directly to Tymly regarding your experience with the platform.
9. Intellectual Property
Tymly Forms, including its software, branding, designs, workflows, interfaces, documentation, and related intellectual property, is owned by or licensed to Tymly.
Participation in the beta does not transfer ownership of Tymly Forms or grant participants any ownership interest in the software.
You receive a limited, temporary, non-exclusive, non-transferable right to access the beta solely for authorized testing and evaluation.
You may not copy, sell, sublicense, reverse engineer, reproduce, commercially redistribute, or attempt to create a competing derivative product from Tymly Forms except where applicable law expressly permits otherwise.
10. Beta Access
Tymly may suspend, restrict, or end beta access at any time, including for:
-
security concerns;
-
misuse of the platform;
-
violation of these Beta Terms;
-
technical or operational reasons;
-
completion of the beta program; or
-
transition to commercial release.
Participants may stop participating in the beta at any time.
11. Fees
Unless otherwise stated in writing, participation in the private beta is provided without charge.
Future commercial use of Tymly Forms may require a paid subscription.
Participation in the beta does not guarantee any particular future price, discount, subscription plan, feature set, or amount of free access unless Tymly specifically agrees to those terms in writing.
12. No Guarantee of Commercial Release
Participation in the beta does not guarantee that Tymly Forms will be commercially released, that every beta feature will remain available, or that the platform will continue operating in its beta form.
Tymly may modify the product, pricing, functionality, or launch plans based on testing and business needs.
13. Disclaimer of Warranties
To the extent permitted by law, the beta version of Tymly Forms is provided on an “as is” and “as available” basis for testing and evaluation.
Tymly does not guarantee that the beta will be uninterrupted, error-free, complete, or suitable for every practice or use case.
Artificial intelligence may generate incomplete, incorrect, or inappropriate suggestions. All generated content must be reviewed by the participating provider before use.
14. Limitation of Liability
To the maximum extent permitted by applicable law, Tymly will not be liable for indirect, incidental, special, consequential, or punitive damages arising from participation in the beta program.
Participants remain responsible for reviewing and approving any documentation generated through Tymly Forms before relying upon, signing, submitting, or otherwise using it.
Nothing in these Beta Terms limits liability where such limitation is prohibited by law.
15. Privacy
Information collected through the beta application and beta program will be handled in accordance with the Tymly Forms Privacy Policy.
Participants should review the Privacy Policy before submitting an application or participating in the beta.
16. Changes to These Terms
Because Tymly Forms is under development, we may update these Beta Terms as the beta program evolves.
If we make material changes affecting current participants, we may provide notice through email, the platform, or another reasonable method.
Continued participation after updated terms become effective constitutes acceptance of the revised Beta Terms.
17. Governing Law
These Beta Terms are governed by the laws of the State of Louisiana, without regard to conflict-of-law principles.
Any dispute relating to these Beta Terms will be subject to the applicable courts located in Louisiana unless otherwise required by law or agreed to in writing.
